1. What’s In A Kit?
First,
all packages contain the complete kit:
• Notices
• Acknowledgements of Receipt
• Privacy Posters
• Red and Green Chart Label Stickers
• Administrative Support Forms
• Business Associate Agreements
• Workforce Training Posters
2. How Many Do I Get?
There are three sizes of Kit,
based on the number of Notices you get in the Kit.
• The Small Kit has 500
Notices,
• The Medium Kit has 1000 Notices
• The Large Kit has 2500 Notices.
(Every Notice comes with the required Acknowledgement of Receipt. There is
a full listing of quantities at the bottom of this page.)
3. Basic or Deluxe?
The main difference between Basic and Deluxe kits is that the Basic kit
contains the 4-page Basic Notice, while the Deluxe kit contains the
6-page Deluxe
Notice. Both notices contain the same information, but the type is
larger in the Deluxe
Notice and there’s room for more personalization information about
your Practice on it.

4. Two Questions to Help You Decide:
1) The first question below will help you decide if you need
the Basic or Deluxe kit.
2) The second question will help you decide
if you need a small, medium or
large kit.
Question #1 – Basic or Deluxe
Notices
The Basic Notice contains the same information as the Deluxe Notice. However,
the Basic is printed on 4 pages while the Deluxe is printed on 6 pages.
The print on the Deluxe Notice is larger, and there is more room for information
about your Practice.
• If your practice sees mostly older patients you need the Deluxe Notice (They’ll
appreciate the larger type.)
• If more than two doctors will be included on your notice, then you need the
Deluxe Notice
• If it is important to include credentials, boards and affiliations in your
personalization information, then you need the Deluxe Notice
• If more than one office location will appear on your notice, then you need
the Deluxe Notice
• If none of the above criteria apply to your practice you need the Basic Notice
Question #2 – Small, Medium or Large
We recommend that you order a minimum three month supply.
The volume you need is equal to the following: The number of new patients you
expect to see in the next three months plus old
established patients returning to the office for the first time since you've
implemented your Notice program.
• If you see 500 patients in the
next 3 months you need a Small Kit
• If you see 1000 patients in the
next 3 months you need a Medium Kit
• If you see 2500 patients in the
next 3 months you need a Large Kit
(Remember that EVERY patient, new or established,
needs to receive the notice on their first visit once you've implemented your
HIPAA compliance program.)
