You’re
required to give your patients a Notice of Privacy Practices.
At that time you are also required to
request a signed receipt from the patient acknowledging
that they have received the Notice. There are actually
four forms on the AR, working together as part of the HIPAAeasy
System.
1. The Acknowledgement of Receipt
- The
language on our AR is simple, clear, unintimidating and
unambiguous. The patient will not feel like he is
being asked to sign something he does not understand, saving the staff
the time-consuming effort of explaining.
2. Documentation Of Inability To Obtain Receipt Acknowledgement
Signature -
This documentation is required if a patient is unwilling or unable to sign
an Acknowledgement of Receipt. No other AR contains this form. (This saves
you the work and cost of placing additional sheets in the record.
3. Summary of PHI Communications
and Disclosure Restrictions – Under
HIPAA your patient may place restrictions on the disclosure of their Protected
Health Information (PHI). Further the patient may place restrictions on the
Communications you may make to him, such as certain phone numbers and mailing
addresses. HIPAAeasy’s AR contains a form where your patient’s
PHI restrictions are clearly noted. When placed in the patient’s chart,
the AR becomes an easy reference to your patient’s PHI restrictions.
4. PHI disclosure accounting log – when
a disclosure must be logged, the presence of the log form
right on the
AR means that there is one less piece
of paper to search for. Further, even if you never need to log any disclosures,
the presence of the Log form is proof of your intent to use such a form
should the need arise. (Without such proof, an attorney
might argue that you may have
made disclosures and not logged them.)The AR that comes with the Basic
Notice is a single detachable page embedded right in
the notice.
The AR that comes with the Deluxe Notice is a separate two-part NCR duplicate
form. It is personalized to match the Notice. The two-part carbonless form
automatically generates copies for both the patient and practice. The patient's
copy of the Receipt contains a summary of the Notice creating a layered Notice
as encouraged under HIPAA. The office copy of the form features the four sections
outlined above.
Both versions of the AR come with holes already punched at the top for easy
incorporation into the chart
The AR is an important part of the HIPAAeasy system.
When a patient requests special PHI restrictions the HIPAAeasy
Red Chart Label is placed
on the patient’s chart. The next time the patient’s chart is referenced,
the Red Chart Label alerts the staff that special PHI restrictions are in place.
The staff then knows to look inside the folder for the Acknowledgement of Receipt
to learn exactly which special restrictions are in place.
